Position Title: Chemical Health Coordinator

Introduction:

 

This position within HAP will partner with Northwest Hennepin Human Services Council (NWHHSC) and the Lao Assistance Center of MN (LACM) in a program called: Southeast Asians Living Chemically Free, which serves the Hmong community in NW Hennepin County region.

The goals of the program are to:

  • Increase Awareness regarding chemical abuse and addiction
    • Build the capacity of the Southeast Asian and mainstream community in this area to better serve Hmong.
    • Address chemical health issues for Southeast Asian individuals affected by substance abuse, by increasing awareness, building treatment model concepts with selected mainstream providers and supporting access to appropriate culturally sensitive community resources.
  • Build Trusting Relationships
    • Build trusting relationships as Chemical Health resources with the Southeast Asian community
    • Engage and partner with community leaders about Chemical Health issues and solutions

Primary Function of the Position

 

Under the direction of the Director of Youth and Family Services Programs at Hmong American Partnership, and the NWHHSC Chemical Health Program Coordinator, this position will serve the Hmong community by coordinating all activities required in the program, working as a team with two part-time staff members who will conduct education sessions in groups, and mentor participants individually.

 

Major Position Responsibilities include:

·         Provide vision and leadership for  the SE Asians Living Chemically Free Chemical Health program

·         Assess the chemical health care needs of the Hmong community

·         Build and maintain relationships and linkages with community resources which provide chemical health care to the Hmong community

·         Provide chemical health information, referrals, and /or advocacy to Hmong clients and families as appropriate

·         Work in partnership with other Chemical health workers and related professional and paraprofessionals in the performance of job responsibilities

·         Plan and coordinate community chemical health outreach, education ,and support group activities which would be delivered by two part-time professional staff members

·         Coordinate and manage contracted services

·         Help with participant recruitment and outreach as needed

·         Work with partners to meet the grant requirements:

ü fully understand the program goals, objectives, timelines and outcomes;

ü develop and plan program activities according to the grant timelines;

ü monitor the program objectives to ensure the effectiveness of the services delivered;

ü collect data about clients through surveys, interviews and case history;

ü prepare all documentation for grant reports as requested;

ü participate in Chemical Health evaluation and outcomes discussions

·         Conduct reporting and data collection as required

·         Manage and track the program budget and expenses

·         Maintain all program and financial records

  • Attend staff  and DHS quarterly meetings and trainings
  • Meet with supervisor as requested
  • Meet with team as needed

Skills, Knowledge and Abilities

·         Effective leadership style that promotes positive customer services that effectively connects the participant with the needed services.

·         Effective bi-lingual communication skills – both written and oral in English and Hmong.

·         Ability to interpret legislation and regulations, and effectively integrate and communicate these rules and regulations with programs and staff.

·         Strong interpersonal skills and ability to work as a team

·         Good computer skills including word processing, data entry and email

 

Job Qualifications:

 

 

Education:   B.A. /B.S. degree in Health Care, Social Work, or Psychology.

                      

Experience: Previous experience working in a chemical health or other health related capacity with Hmong families.  

Knowledge: Knowledge of Hmong culture and issues facing Hmong families is also required.

 

General:   Fluent and literate in Hmong and English. 

 

·         Ability to establish rapport with and gain respect from Hmong Youth and adults is essential.

·         A commitment to building the strengths of Hmong families is necessary.

·         Position requires flexible scheduling including occasional weekends, late evening events and travel.

·         Must have a reliable car, a valid driver’s license, automobile insurance and the willingness to use personal vehicle for work related travel.

Position Title:  Hmong Youth Pride Site Tutor

Status:  Part-time, contract, start date:  ASAP

Primary Functions of the Position

 
Under the direction of the HYP Coordinator, this position is responsible for assisting in planning and implementing tutorial services at several school sites, teaching Hmong cultural enrichment lesson plans, supervising educational fieldtrips and athletic events/games to young participants in 2nd to 8th grade enrolled in Hmong American Partnership’s Hmong Youth Pride after-school program, promoting positive working relationship between the HYP program, school partners, the community and Hmong American Partnership staff.

 

Major Position Responbilities:

 

  • Completes St. Paul Public School tutoring training and attend trainings and in-services as needed.
  • Provides 1:1 reading tutorial to Hmong students as assigned by the school staff
  • Tutor students in basic elementary subjects
  • Tutor ELL students with basic elementary subjects
  • Lead small group activities that relate to planned curriculum
  • Create and implement lesson plans and activities for students
  • Assist HYP staff with program activities and events
  • Assist in planning and supervising fieldtrips and program activities
  • Assist in volunteer and HYP program recruitment
  • Manage volunteers on site
  • Assist in transporting/distributing program information & materials
  • Maintains accurate records, case notes and reports of program activities
  • Successfully distribute and collect evaluations from teachers, students, partners and schools
  • Assist with summarizing data from evaluations and surveys 
  • Maintains accurate weekly schedule of hours and update the weekly schedule as plans change
  • Provide transportation and interpretation services for parents during parent-teacher conferences
  • Conducts home visits as assigned by the HYP coordinator
  • Serve as the liaison for your site and Hmong Youth Pride

 

Job Qualifications:

 

Experience: 

  • 18 years of age or older
  • 1 yr experience or more working with children between the ages of 8 to 14 years of age
  • Experience in lesson planning
  • Experience planning and organizing events, activities and fieldtrips
  • Experience leading small group activities 

     

    Education: 

    • High School diploma or some college education;  Bachelors Degree is a plus!

     

    Knowledge: 

    • Proficient knowledge of Child Development is required.
    • Must be familiar with school settings & educational systems

     

    Skills: 

    • Must be fluent and literate in Hmong and English to be able to work with ELL students
    • Must be able to work with Hmong children, parents and families; specifically with low income families
    • Must be familiar with Hmong culture and traditions

     

    Abilities: 

    • Must enjoy working with children. 
    • Must have a desire and the ability to improve academic standing, self-esteem, confidence and pride of Hmong Youth Pride participants. 
    • Must be able to work with ELL students who have entered the U.S. only recently. 
    • Must be able to understand Hmong-American experience.
    • Must be able to cater tutoring to needs of elementary/middle school aged participants.
    • Must enjoy outdoor activities with children (sports, camping, roller skating, etc.)
    • Must be athletic.

     

    General:  Position requires having a reliable car, a valid driver’s license, automobile insurance and the willingness to use personal vehicle for work related travel.  Must be flexible and have the ability to work non-standard hours & some weekends.
