HAP’s Diabetes Prevention program is a FREE wellness program designed to delay/prevent Type 2 diabetes and improve overall well-being. It is presented by trained lifestyle coaches in Hmong and English and uses methods and materials that are culturally appropriate for Hmong and other Southeast Asian communities. The program curriculum is evidence-based, developed by the Centers for Disease Control and Prevention (CDC), and includes helpful information on balanced diets, portion control, physical activity, and more.
By participating, you will learn about Type 2 diabetes risks and prevention, nutrition and general health. You will also benefit from guided physical activity that will help you lose 5% – 7% of your body weight and gradually gain 150 minutes of physical activity. The group physical activity instruction will help you build stamina, improve your mobility, and gain confidence! We even provide essential tools and support to help you achieve your goals.
Eligibility & Duration
- Adults of all ages who are at high risk for Type 2 diabetes or prediabetes are invited to participate. Instruction is provided in both Hmong and English.
- The program lasts one year and includes 16 weekly, four bi-weekly, and six monthly meetings.
